How to Access Services

How to Access Care Coordination

If you test positive at The Damien Center, you are automatically referred to a Care Coordinator when you are diagnosed.

If you are already positive and would like to access our services, call the receptionist to schedule an appointment with an available Care Coordinator.

This first meeting with a Care Coordinator is called an intake. An intake is an assessment by a Care Coordinator of every area of a person's life. This assessment allows a Care Coordinator to prioritize your needs and help you access the proper services. An intake typically takes two hours.

If you are able, please bring the following to your intake:
  • Photo ID (like a driver's license)
  • Insurance cards
  • Proof of income (like pay stubs, social security reward letter, or INET statement)
  • Proof of residency (like a lease, utility, or any other mail you might receive)
If you don't have one or more of the above, don't worry! Your care coordinator will help you whether you have them or not. These documents just help our care coordinators get you the proper services, and they'll help you get any documents you may not have.

If you were a Damien Center client but haven't seen us for two or more years, we are always happy to see you again! However, you'll need to schedule an appointment to be reassessed because your needs may have changed during the time that you were away.

Wait times for an intake or reassessment can be anywhere between a couple days to two weeks. This time varies depending on a client's needs and a Care Coordinator's availability.
