How to Access Services

Services

Our Care Coordinators will help you with all of your needs. Some common services our Care Coordinators provide are listed below. This list is not comprehensive - these are just a few of the things our Care Coordinators can do for you.

Health insurance
Our Care Coordinators will help you enroll in an insurance option that best fits your situation and need.

Referrals for medical care
Our Care Coordinators can help you find a primary care physician. They can also help you find a doctor or clinic for infectious diseases, oral health, dental health, mental health, or substance abuse.

Medication access
Your Care Coordinator can help you enroll in a patient assistance program to help you pay for your medications.

Comprehensive Risk Counseling and Services (CRCS)
We have a comprehensive risk counselor on-site that can help you with things like grief a new HIV diagnosis. There are also support groups led by our comprehensive risk counselor.

Housing and emergency financial assistance
Care Coordinators can help you find housing programs and apply for rent & utility assistance, like HOPWA (Housing Opportunity for People With AIDS) and DEFA (Gregory R. Powers Direct Emergency Financial Assistance). To apply for HOPWA and DEFA, clients must get a referral from a Care Coordinator from any site.

Transportation
Our Care Coordinators can access bus passes to help you get to your appointments for medical care and other needs. Unfortunately, access to these bus passes are limited because of the large need.

Nutritional
We have an on-site food pantry. Access to the food pantry must be made through your Care Coordinator, and access is limited to one (1) visit per two (2) weeks. In an emergency, our Care Coordinators can access vouchers.

Legal assistance
Our Care Coordinators can refer you to advanced directives, like:
  • Last will & testament
  • DNR (Do Nor Resuscitate)
  • Living will
  • Power of attorney
  • Healthcare
  • Legal representation
Our Care Coordinators can also provide referrals for other legal needs.

Employment
Our Care Coordinators can link clients to employment services located within The Damien Center (like Ticket to Work) and in the community. Services include, but are not limited to, resume writing, interview skills, and professional attire.

Social support
Many clients choose not to disclose their HIV status to anybody but their Care Coordinator. Care Coordinators know that sometimes, you just need to get something off your chest. They'll be there for you with whatever you need - even if it's just a talk.
